Nurse Navigator Womens( Job Number: 00323-10781) Work Location: United States-Kansas-Wichita-Wesley Medical CenterSchedule: Full-timeDescription Demonstrates the ability to adapt technique/t! reatment to age of population served, including newborn, pediatric, adolescent, adult and geriatric patients, while providing care/service with respect to cultural differences and spiritual needs. Build relationships with patients and physicians, to coordinate a plan of care including appointments, transportation, education, provision and/or enablement of support services and representation within the multidisciplinary care environment Assumes responsibility and accountability for the management of resources to achieve efficient, high quality outcomes for each patient including support for interdisciplinary and cross facility collaboration e.g. fetal boards, and communication with the referring physician On an aggregate level, tracks performance of the program in line with HCA, division and facility goals (growth, quality, practice guidelines, etc.) and identify opportunities to streamline care practice, through development of evidence-based guidelines Serves as a liaison b! etween the patient and family, primary care physician, interna! l and external care providers, specialists, support network members e.g. social workers, and the wider healthcare community; Conducts internal and referral for external outreach and marketing.
May include health fairs, screenings and other special events Assesses and initiates referrals to hospital and community resources to connect patients as needed Assesses the medical, social, and psychosocial and other care needs of the patient and family Provides appropriate teaching, outreach, and education to patients and families to ensure the patient is empowered to manage his or her own health Streamlines appointments and paperwork by helping patients with scheduling appointments and preparation Establishes algorithms, documents, and formalized processes for transition in commonly followed care pathways Coaches and helps patients to remove barriers with issues of insurance, transportation, child care, financial resources, language so they may focus on getting the care th! ey need Initiates referrals to hospital and community resources to connect patients with resources and support systems Conducts follow-up conversations as needed with all patients to communicate concerns, changes, or social needs in patient health to appropriate MD or other care providers Supports and conducts outreach to referrers, providers, other medical professionals and the community Organizes and attend patient care planning conferences and other meetings as necessary Tracks and document interventions and outcomesQualifications Required Registration, Licensure or Certification Current Licensure in the State of Kansas as a Registered Professional Nurse (RN) Current Basic Life Support (BLS) verification course, as specified in policyPreferred Registration, Licensure or Certification Certificate of qualification, Advanced Registered Nurse Practitioner according to the Kansas Nurse Practice Act Experience in program development Experience as a nurse navigator and/or patie! nt educatorRequired Education or Experience Bachelor`s Degree in Nursin! g Minimum of five years Women`s, NICU or Pediatric nursing experiencePreferred Education or Experience Masters degree in Nursing or Healthcare AdministrationRequired Knowledge and Skills Ability to utilize the nursing process in the provision of nursing care including, but not limited to, administering treatments and medications, interpreting diagnostic tests, formulating a plan of care, collaborating with other health care providers and providing education to patients and/or significant others Demonstrated ability to communicate effectively Knowledge of community resources Possess personal computer skills Ability to multi-task, delegate and prioritize Possess problem-solving and critical thinking skillsPreferred Knowledge and Skills Ability to communicate in Spanish
