We have an outstanding opportunity to work for a fast-growing infusion therapy / specialty pharmacy organization, and are seeking a patient-focused PRN Home Infusion Nurse with recent experience in home infusion (or ER / ICU with multiple peripheral line starts daily).
Job Description
As a PRN Home Infusion Nurse for AxelaCare, you will infuse medications on a regular basis in patient’s homes, while leveraging new, technology-driven solutions which allow physicians an! d nurses to easily capture data related to the treatments of t! hese rare and complex diseases and patients' responses to them - ultimately improving patient outcomes. This includes IV placement, phlebotomy and medication administration via peripheral infusion device, PICC line, implantable port and/or Central Venous Catheters.
Job Responsibilities
As a PRN Home Infusion Nurse for AxelaCare, you will establish and maintain positive clinical rapport with all patients and their families while maintaining contact and monitoring the effectiveness of the medication treatment. Additional responsibilities include:
Assessing the patient’s pain and environment within their home setting Developing a plan of care and educating the patients and their families on their treatment. Establishing and maintaining all clinical documentation including Plans of Care/Treatment (POC/POT), clinical summaries, medication changes and updates for the AxelaCare pharmacy Establishing collaborative relationships involving all agencies Delivering servic! es on behalf and/or in conjunction with AxelaCare Providing input regarding strategic direction and tactical solutions for your patients

Job Requirements
To be a PRN Home Infusion Nurse for AxelaCare, you must be willing to work with most ages and diagnosis of patients in their homes who are receiving infusion therapy medication. We need someone who can communicate effectively, employ excellent project management skills and the ability to work both one-on-one and in group settings. Additional requirements include:
Graduation from an accredited 4 year nursing program Minimum of 3 years experience and proficiency in infusion therapy. Infusion in home health setting is highly desirable. MUST HAVE a high level of proficiency with the use of technology, thru exposure to EMR, use of data phones or tablets with WiFi access, and proficiency in Microsoft Office Suite (Excel and Word) Experience with Cardiac, Pediatric and/or Maternal/Chil! d patient setting is highly desirable Experience in HIPAA, OSHA and/or ! JCAHO is preferred, but not required Ability to work with an interdisciplinary team of professionals including clinical, pharmacy, physicians and outside infusion nursing agencies Exceptional interpersonal, listening, organizational, teaching and critical thinking skills as well as maintaining a professional appearance Ability to pass a criminal background check
