Assesses each assigned patient referral to identify discharge planning needs, and collaborates with patient, family, significant other, and members of the health care team to develop and implement discharge plans and perform utilization review functions based on hospital policy and procedure. Assesses, develops, implements and monitors a comprehensive plan of care through an interdisciplinary process; providing both the clinical and psychosocial approach through the continuum of care. Balances individually identified patient and family needs with the efficient and cost-effective utilization of resources. The Case Manager in department 768 provides care for newborn/pediatric patients from birth to 11 years of age; adolescent patients between 12 to 17 years of age; adult patients between 18 to 64 years; and geriatric patients 65 years and older.Education: Graduate of an accredited school of nursing required.Bachelor’s degree preferred.Licensure/ Certifications: Current Kansas RN License and BLS required.Experience: Excellent interpersonal and organizational skills required.Ability to work effectively with health care team.Knowledge of computers, statistics and managed care concepts preferred.An Equal Opportunity Employer (M/F/H/V).
